Disc Treatments in Marlton and Collingswood, NJ
Disc-related pain can make simple movement difficult. Sitting, bending, lifting, standing, driving, working, exercising, sleeping, or travelling may become more difficult when a damaged, irritated, or painful spinal disc contributes to back or neck pain, nerve irritation, or symptoms that travel into the arms or legs.
At Peak Spine & Sports Medicine, we evaluate whether a spinal disc is truly contributing to pain and whether a targeted disc procedure may help. Many patients have disc changes on imaging, but not every worn, bulging, or damaged disc is the main source of pain. A careful evaluation helps connect symptoms, imaging, exam findings, and daily limitations before a treatment plan is recommended.
We provide opioid-free, interventional pain care with an emphasis on education, outcomes, and quality of life. Dr. Milind Patel, Dr. Jill Kalariya, Brian Brown, PA, and our interventional pain management team deliver care through a concierge model designed to ensure clarity, organization, and a personalized experience.

What Are Disc Procedures?
Disc procedures are minimally invasive spine treatments used to diagnose or treat selected disc-related pain.
Spinal discs sit between the vertebrae and help absorb pressure, support movement, and act as a cushion. When a disc becomes damaged, irritated, torn, inflamed, compressed, or herniated, it may contribute to pain in the back, neck, arms, or legs.
Disc procedures may be considered for selected patients with:
- Discogenic low back pain
- Contained disc herniations
- Bulging discs
- Annular tears
- Degenerative disc disease
- Disc-related nerve irritation
- Chronic back pain that has not improved enough with conservative care
- Painful vertebral compression fractures, in the case of vertebroplasty or kyphoplasty
The right procedure depends on the diagnosis. Disc Decompression and Lumbar Discography focus on disc-related pain. Vertebroplasty and Kyphoplasty treat vertebral compression fractures rather than the disc itself, but they are often grouped with advanced minimally invasive spine procedures.
Disc Decompression
Disc Decompression may be considered when a contained disc herniation or disc-related pressure is irritating a nearby nerve.
Percutaneous Disc Decompression is designed to remove or reduce a small amount of disc material through a minimally invasive approach. In carefully selected patients, reducing disc pressure may help reduce nerve irritation and radiating symptoms.
Disc Decompression may be discussed for selected patients with:
- Contained herniated discs
- Disc-related nerve irritation
- Sciatica or radicular pain
- Back pain with leg symptoms
- Neck pain with arm symptoms in selected cases
- Symptoms that match imaging findings
- Pain that has not improved enough with conservative care or injections
This procedure is not appropriate for every disc problem. Large, extruded disc fragments, severe stenosis, instability, progressive weakness, or other structural concerns may require a different evaluation.
Lumbar Discography / Provocative Discography
Lumbar Discography, also called Provocative Discography, is a diagnostic procedure used in carefully selected cases to help determine whether a spinal disc is a source of pain.
During a discography, contrast is injected into the disc while the physician evaluates whether the pressure reproduces the patient’s usual pain pattern. Imaging may also show whether the disc has internal damage, leakage, or annular disruption.
Discography may be discussed when:
- Chronic low back pain appears disc-related
- Imaging shows multiple possible disc levels
- The pain source remains unclear after other evaluations
- The results may affect treatment planning
- A disc procedure or surgical decision is being considered
Lumbar Discography is not used for every patient with back pain. It is a diagnostic tool, not a treatment. Peak Spine & Sports Medicine considers discography only when the information may meaningfully guide the next step in care.
Vertebroplasty / Kyphoplasty
Vertebroplasty and Kyphoplasty are minimally invasive spine procedures used to treat painful vertebral compression fractures.
These procedures do not treat the spinal disc. They treat the vertebral bone, which can collapse or fracture due to osteoporosis, trauma, cancer-related weakening, or other conditions. They are included in this disc procedures section because they belong to the broader category of advanced minimally invasive spine care.
During Vertebroplasty, medical bone cement is injected into the fractured vertebra to help stabilize the bone.
During Kyphoplasty, a small balloon may be used first to create space within the fractured vertebra before cement is placed.
Vertebroplasty or Kyphoplasty may be considered for selected patients with:
- Painful vertebral compression fractures
- Osteoporosis-related spinal fractures
- Severe focal back pain from a confirmed fracture
- Pain that limits standing, walking, breathing deeply, or daily activity
- Fracture pain that has not improved enough with conservative care
These procedures are not used for general back pain, herniated discs, or degenerative disc disease. A careful evaluation and imaging review help determine whether fracture stabilization may be appropriate.
Who May Be a Candidate for Disc Procedures?
Disc procedures may be considered when symptoms, imaging, exam findings, and prior treatment response suggest that a disc or vertebral fracture is a meaningful pain source. A consultation helps determine whether a disc procedure is appropriate or whether another treatment category may better match the pain source.
Patients may be candidates if they have:
- Chronic low back pain
- Discogenic pain
- Contained herniated disc
- Disc-related nerve irritation
- Annular tears
- Pain that worsens with sitting, bending, lifting, or spinal loading
- Pain that travels into the leg or arm from disc-related nerve irritation
- Painful vertebral compression fracture
- A clear functional goal, such as walking farther, sleeping better, working comfortably, exercising, travelling, or reducing severe flare-ups
- Pain that has not improved enough with conservative care

Disc Procedures Marlton, NJ – FAQs
What are disc procedures?
Disc procedures are minimally invasive spine treatments used to diagnose or treat selected disc-related pain. They may include Disc Decompression, DISCSEEL, IDET, and Lumbar Discography.
What is discogenic pain?
Discogenic pain is pain that appears to come from the spinal disc itself. It may be related to annular tears, disc damage, degeneration, inflammation, or internal disc disruption.
Is every bulging disc painful?
No. Many people have bulging or degenerative discs on imaging without significant pain. Treatment should focus on the disc only when symptoms, exam findings, and imaging support it as the likely pain source.
What is Disc Decompression?
Disc Decompression is a minimally invasive procedure that may reduce pressure within a selected disc. It may be considered when a contained herniated disc is irritating a nearby nerve.
What is Lumbar Discography?
Lumbar Discography, also called Provocative Discography, is a diagnostic procedure used to help determine whether a spinal disc is a source of pain. It is typically reserved for carefully selected cases.
Is discography a treatment?
No. Discography is a diagnostic test. It may help identify whether a disc is painful and whether that information should guide future treatment planning.
Are Vertebroplasty and Kyphoplasty disc procedures?
They are not true disc procedures. Vertebroplasty and Kyphoplasty treat painful vertebral compression fractures, not spinal discs. They may be included in this category because they are advanced minimally invasive spine procedures.
What is the difference between Vertebroplasty and Kyphoplasty?
Both procedures use bone cement to stabilize a painful vertebral compression fracture. Kyphoplasty typically uses a small balloon first to create space within the fractured vertebra before cement is placed.
Who may benefit from Vertebroplasty or Kyphoplasty?
These procedures may be considered for selected patients with painful vertebral compression fractures that have not improved enough with conservative care and are confirmed on imaging.
Can disc procedures help sciatica?
Disc procedures may help selected patients when sciatica is caused by a contained herniated disc or disc-related nerve irritation. Candidacy depends on symptoms, imaging, exam findings, and prior treatment response.
Do disc procedures replace spine surgery?
Disc procedures may help selected patients avoid or delay surgery, but they do not replace surgery when severe compression, instability, progressive weakness, or another serious structural problem requires surgical evaluation.
Does Peak Spine & Sports Medicine treat pain with opioids?
No. Peak Spine & Sports Medicine is an opioid-free pain management practice. The team focuses on education, interventional procedures when appropriate, and targeted non-opioid treatment strategies.
Does Peak Spine & Sports Medicine accept insurance?
Peak Spine & Sports Medicine accepts insurance and primarily works with PPO and POS plans that have out-of-network benefits. The practice does not participate with EPO, HMO, Medicare, or Medicaid plans. The team can help determine whether benefits may apply.
